Accounts must use accurate information; fake profiles will be terminated.

2. Appointment Policy

Appointments require 48-hour confirmation via phone/email.
Cancellations under 24 hours may incur a $50 fee (emergencies exempt).
Late arrivals beyond 15 minutes may require rescheduling.
New patients must complete medical history forms prior to treatment.

3. Payments & Insurance

Full payment is due at time of service unless prior arrangements are made.
We accept Visa, MasterCard, CareCredit, and HSA cards.
Insurance claims are filed as a courtesy; patients are responsible for uncovered charges.
Returned checks incur a $35 processing fee.

4. Patient Responsibilities

Provide accurate medical/dental history including medications and allergies.
Notify staff of any changes in health status before procedures.
Parents/guardians must accompany minors under 16.
Follow all post-treatment care instructions to avoid complications.

5. Privacy Policy
